Evaluation

The first step in the effective treatment program is a complete evaluation.  Our staff uses a variety of standardized tests that give objective scores, as well as informal measurements that can be helpful for developing treatment plans. If you have recently received an assessment (within the last six months) in another setting, please bring that information with you or fax to office at the time of your evaluation appointment (801)308-0401.
